There is no mail-in or online registration for the 2015 season, so plan now to attend one of the two remaining walk-in registration sessions.
Important note: There is no photocopy machine available at registration. You must have your copies ready and made ahead of time.
WHAT TO BRING:
- Completed Player Application
- 3 different proof of residency documents, *bring copies to submit
- Original birth certificate for DOB verification -NEW PLAYERS ONLY
- Copy of birth certificate for league files – NEW & RETURNING PLAYERS
- Completed Financial Agreement (1 per family)
- Completed Parent/Guardian Contract (1 per family)
- Completed Medical Release Form (1 for each child)
- Check or credit card for payment
